Clinical Analysis of Biliary Stone



Abstract
It has been frequently reported that clinical features of biliary stone disease in Koreans is different from that of Occidentals. These differences were thought to be due to inherent racial differences and acquired living habits. Recently,
Koreans
socioeconomic living has improved and become more like Occidentals. To evaluate whether the pattern of biliary stone disease has change or not, we analyzed 206 cases of biliary stone patient who had been treated surgically in the Department of
Surgery
of Hae Sung General Hospital from January 1991 to December 1994.
@ES The results obtained were as follows:
@EN 1) The peak incidence was noted in the 4th decade and the male to female ratio was 1:1.4.
2) The major complaint of these patients was RUQ abdominal pain(83%).
3) Major positive laboratory findings were an elevated leukocyte count(24%), SGOT(29%), SGPT(30%), and alkaline phosphatase(28%).
4) The detection rate using ultrasonography was 89.7% in diagnosing hepatobiliary stones.
5) The location of the stone was in the GB in 130 cases(63%), both GB and CBD in 22 cases(11%).
6) The pathologic diagnosis was chronic cholecystitis(79%), GB empyema(5%).
7) The most common operative procedure was cholecystectomy(74%) and cholecystectomy with T-tube choledochotomy was done in 36 cases(17%).
8) The frequency of laparoscopic cholecystectomy was markedly increased from 1992(16%) to 1994(62%)
9) Bacterial culture was performed in 83 cases and the positive culture rate was 41%. The major organisms were E. coli(48%), streptococcus(11%), Klebsiella(9%), and Enteococcus (9%).
10) Postoperatively, there were pulmonary complication(3.4%), wound infection(2.9%), and bile leakage(1.9%). In laparoscopic cholecystectomy, there 3 cases of cases of postoperative complications: CBD rupture(1 case), bleeding(1 case), and bile
leakage(1 case).
11) Associated diseases were diabetes mellitus(20%), hepatitis(7%), clonorchis sinensis(5%), malignant cancer(5%).
